ArenaSpy - From the GameSpy people, it replaces Q3's server lister with a much improved GameSpy-style one. A must have. Unfortunately it's only for win32 because they made a DLL instead of using the Q3VM.
Rocket Arena 3 - Great modification for Quake3. Two players go in, all full weapons, no armor, no health, etc. One is left; next player up. Great custom maps, mp3 player built-in, the works. Better than the original Rocket Arena??
Threewave CTF - Better CTF than the half assed CTF that comes with Q3. By the original makers of CTF.
Western Quake 3 - Wild West Total Conversion. Extremely well done.

We're not spectacular players or anything, but me and anon98 have lots of fun playing 1v1, and have had some really close matches. These aren't the best ones since we don't have demos recording all the time, but they are still entertaining to watch:
